HIPAA NOTICE OF PRIVACY PRACTICES

Xthetics Med Spa is committed to protecting the privacy of your Protected Health Information (“PHI”). This Notice explains how we may use and disclose your health information and your rights regarding that information.

Request restrictions on certain uses and disclosures.
Request confidential communications at an alternative phone number or address.
Request an accounting of certain disclosures.
Receive a paper copy of this Notice.
Receive notification of a breach of unsecured PHI when required by law.
Certain health information, including information protected under 42 C.F.R. Part 2, may receive additional federal privacy protections.
Our Responsibilities
We are required to protect the privacy and security of your PHI, provide you with this Notice, follow the Notice currently in effect, and notify you of certain breaches as required by law.
Questions or Privacy Complaints
If you have questions or believe your privacy rights have been violated, please contact us. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
